WAIVER INFORMATION

1. I/We, the parents/guardian(s) of the above-named candidate for a position on the Nor’Easters Baseball Club hereby give my/our approval to participate in any and all Nor’Easters Baseball Tryout Activities;

2. The player’s participation indicates that I/We fully acknowledge, understand, and appreciate the risks inherent in playing baseball. I/We further acknowledge that the player is physically fit to play baseball competitively, and he/she is voluntarily participating in the tryout; and

3. I/We hereby waive any claims and agree to hold harmless the Nor’Easters Baseball Club, its managers, coaches, affiliates or associates, from any and all claims arising out of operation of the baseball club, any athletic endeavor or any related activity.
