Information:
We are going to offer all VR High Schoolers the opportunity to get their 2017-2018 athletic physical completed during the school day, during their athletic period. In order to arrange help from physicians, we ask that you sign up ahead of time with the completed Medical history on the 2017 GREEN physical forms. All proceeds will go to the VRHS Sports Medicine program.
Turn in completed history portion with signatures by 5/25/2017.
Online portion of physical can be completed after 5/12/2017.
Fee will be due on or before physical date. Checks payable to LISD Athletics.
