Section XI Misconduct Report - W.Suffolk
This form can be used for reporting to Section XI when:
a) Coach or player are restricted to dugout or you have an unusual situation at your game. (Submit form, no call needed to Section)
b) Coach or player are ejected from game. CALL Section by 9:30am to verify receipt of report. PRINT this report before clicking Submit button. Keep a copy of report for your records.
This EJECTION Report will go directly to Don Webster @ Section XI.
When writing reports do not:
-Recommend a course of action
-Use threatening terms
-Bring up heresay or past experiences unless they are directly related to incident
-Speculate, they usually cannot be proven

ALL DISTRICT GRADING OF 4-5 OR BELOW, PLEASE WRITE COMMENTS.
GRADING SYSTEM (1-10). (1) LOWEST, (10) HIGHEST.
THE FOLLOWING ARE JUST SOME GUIDELINES FOR YOU INFORMATION.
PLEASE DO NOT VOTE FOR SCHOOLS YOU DID NOT WORK OR YOUR OWN SCHOOL DISTRICT.
10 EXCEPTIONAL--provides for an enjoyable environment, team shows sound behavior, able to express views and disagreements in appropriate manner.
8 or 9 VERY GOOD--promotes good sportsmanship, offers very few problems, team conduct on most occasions shows respect.
6 or 7 GOOD--team is usually well behaved, coach sometimes may loose focus in disagreements, team may show restraint in unpleasant situations.
4 or 5 FAIR--sometimes reflects attitude set by coach, appears at times unable to reflect good sportsmanship, and offers at times inappropriate behavior towards umpires.
2 or 3 NEEDS IMPROVEMENT--at times provides difficult environment to work in, shows disrespect towards officials and opponents at times, lack of knowledge of rules, which makes arguments more prevalent.
1 UNSATISFACTORY--offers very little control of team and himself, inadequate playing environment, no knowledge of rules or umpire mechanics, appears to loose control at no apparent reason and unwilling to listen to explanations.
