Incident Report
A report must be submitted after any game in which a player, coach, manager, or other club personnel has been removed by an umpire, giving full names of involved parties. Also, in any other instance for which an umpire believes the attention of the league president is required, a report should be submitted at once. This report is to be postmarked within 24 hours of the incident being reported. The umpire is to keep a copy of this report for his/her own records.
