Player Liability Form 2024 (REQUIRED) All parents are required to fill out the liability form prior to any player practicing with WPYB
Code of Ethics/Conduct For Parents 2024 (REQUIRED) Parents can complete this online version of the code of conduct.
Code of Conduct for Players 2024 (REQUIRED) Parents should review with their child and make sure the player understands to expectations.
Playing Up Release of Liability 2024 This form allows players to play up to the next level for one or more games during the regular season. It only needs to be submitted once per season.
WPYB Incident Report Incident Report Use this form to report any incident that may have occurred during a game. This can be used by umpires, coaches or parents. Without a completed incident report the WPYB Board can take no action on the incident.
WPYB Umpire Rating Form This form allows managers to rate umpires. This form should be used by MANAGERS ONLY.
WPYB Current League Manager Interest Form This form allows CURRENT managers to notify league of plans for following season
Sponsorship Payment This form can be used by new and returning sponsors to pay for their sponsorship package.
2024 Milwaukee Brewer Fundraiser We are sponsoring WPYB night at AM FAM and the Brewers have given the league an opportunity to raise money for WPYB through ticket sales. This is a chance to not only attend a great ball game in some wonderful seats but also support WPYB at the same time.
WPYB Play Up/Play Back for 2025 This allows parents to request their child play up one level early or remain back at current level. Completing this form does not automatically mean a player will move up early, all players will be observed by the board and the manager/coaches will have input as well. PLEASE NOTE: PARENT CONVENIENCE WILL NOT BE ACCEPTED AS A REASON TO EITHER PLAY UP OR REMAIN BACK DEADLINE IS JULY 1 FOR THE FOLLOWING SEASON.
2024 Alternate Registration Form Only one player can be paid for at a time. For families of 3 or more, please contact us at wpyb1960@gmail.com CREDIT CARDS ONLY! Do not use the "electronic check" for payment. There is a $30 NSF for any electronic checks that do not clear. For Returning Players: Registration completed after October 1, 2023 will result in team placement that is in the best interest of the entire league.
