Fundraiser Request Form
Leaders - This form must be filled out and turned in to the treasurer for approval at least 7 days prior to start of the fundraiser.
Receipt Turn In Form
Leaders - This form is to be filled out and turned in the the treasurers with receipts from items purchased. (This is not for receipts that you write to parents for team funds.)
Withdrawal Form
Leaders - Please use this form when withdrawaling monies from your team account.
Team Funds Turn In Form
Leaders - Please fill out this form and turn it in to the treasurers along with the corresponding receipts that you have written.
Uniform Balance Turn In Form
Leaders - This form is to be used for all monies that is to go towards uniform balances.
Misc. Deposit
Leaders - This form is to deposit only Misc. items. Such as Leader fees, NSF, etc. (This is not for team funds or Uniform Balances.)
Write-Up Form
Please fill this form out and keep it untill a visit with the Board is needed.
