NEW JERSEY YOUTH SOCCER CONCUSSION PROTOCOL
NJ Youth Soccer has adopted the US Youth Soccer Concussion Protocol for its events (e.g. ODP and State Cups). We encourage member clubs to utilize the form and related protocol. A link with education and related form appears on our website CLICK HERE

- Pages 1 and 2 are retained by the parent as education and information.
- Page 3 is signed by the appropriate individuals and retained by the team/club.
- Club advised to retain the form for record keeping and reference.
- Referee can advise the coach and recommend that a player is evaluated.
- The coach, who has completed concussion training, is the individual who initiates the protocol.
NJ Youth Soccer shares this additional information to help Leagues, Clubs and parents manage player safety.

(NOTE: Only head coaches and admins have access to the club level portal of US Soccer Connect...assistant coaches can be added by written request to the registrar.)

US Soccer Connect - Adding a player/coach, printing roster, passes & medical releases
Provide this link to your new player and have them register using their existing login info, or create an account if brand new to soccer via twincountysoccer.njyslive.com/home.php
Then, access your US SOCCER CONNECT Portal at:
https://www.secure-sam.com/twincountysoccer.njyslive.com/sam/index.php
To assign a newly registered player to your team:
*login to US Soccer Connect
1. click on your team so your settings page comes up with all players and coaches listed
2. in the bar above, select the "Assign Participant" button
3. a popup window will appear with your newly registered player in the right column
4. click and hold on that player and drag them over to the left column
5. click "save assignments" at bottom...when complete, a green "saved" box will appear, close window
6. player should now be on your roster
7. notify (via email) the registrar and district commissioner (DC) of the player add (put player/coach name and team name in subject line)
8. once they are verified/approved by the DC, a green circle instead of a red circle will appear next to their name in the Portal.
To print new roster:
*login to US Soccer Connect...from your team settings page:
1. click on "manage roster" next to the event you wish to create a roster for ...ex. MOSA Spring 2019, or CJYSA 2018-2019
2. make sure the new player appears on your roster
3. click on "save"
4. click on "print" and a pdf file will be created of your roster
5. player id's are on that (copy/paste into Gotsoccer)
6. print out and keep in binder
COPYING YOUR PLAYER ID #'S INTO GOTSOCCER:
*IN ORDER FOR YOUR NJYS PLAYER ID #'S TO BE ON THE GAME CARD FOR YOUR GAMES AND TOURNAMENTS, YOU MUST CLICK ON YOUR PLAYERS NAMES IN THE ABOVE PORTAL AND COPY, THEN PASTE THEM INTO THEIR GOTSOCCER PLAYER PROFILE.
To print new set of passes/new player pass:
*login to US Soccer Connect...from your team settings page:
1. click on "members" in the menu bar above
2. select "print member cards
3. a pdf file will be created of your roster
4. print passes after checking that passes match roster and all passes have a photo
5. crop/trim passes, then laminate them (passes not valid unless laminated)
To add a new coach:
1. have them create a US SOCCER CONNECT account, then register for the current season, as you normally would, via twincountysoccer.njyslive.com/home.php
2. note: all new coaches must be approved by TCSA Board prior to signing up.
3. notify (via email) the registrar and district commissioner (DC) of the player add (put player/coach name and team name in subject line)
4. go into your US SOCCER CONNECT Portal (and select "Assign Coach" button at top of page
5. move the coach from "Available Coaches" to "Assigned Coaches"
6. click on "Save Assignments"...the coach will be added to your roster when you see a green "Success" banner/box appear at top of box.
7. once they are verified/approved by the DC, a green circle instead of a red circle will appear next to their name in the Portal.
Printing Team Medical Releases for your coach’s binder…
1. in your team page in US Soccer Connect, locate "Print Medical Release Forms For Team Roster"
2. click on "Print Medical Release Forms For Team Roster" to create pdf of med releases
3. print out and keep in binder
4. *(IT IS NOT NECESSARY FOR NJYS TEAMS TO HAVE THEIR MED RELEASES SIGNED BY PARENTS SINCE THEY "ESIGNED" THEM ONLINE ALREADY)*
Applying to the League/Event you wish to participate in...
After all of your players are approved (green dot, not red dot, next to their name on your team page in US Soccer connect), you can register for your league.
This is different than GotSoccer registration and needs to be done so that you may create an official NJYS/USYS roster.
Steps:
-Login to US Soccer Connect
-click on your team "folder" or name -on the page that opens click on "Register for Leagues & Tournaments"... -click on the league/season you are registering for (EDP is listed as CJYSA-EDP)... -follow instructions and complete the league registration requirements.
