1. If you didn't pay online, please mail in your check made payable to:

2. Check out our Tournament Rules.
3. Submit your Team Roster.
4. Submit coach waivers and player waivers
5. Make sure you have a Copy of All Birth Certificates. They must be on hand at all times.
6. Insurance Certificate with Additional Insured listed as:
