WIVL - Medical Treatment & Concussion Agreement

This WIVL/GNBL form must be filled out prior to the season along with registration.  (This replaces the required paper form in previous years.)    THIS MUST BE FILLED OUT EACH YEAR!

Please note that this system requires you to create an account (click "Create an Account" at the very top of the webpage or Login if you already are a member), to authenticate that a parent or legal guardian has electronically signed the Waiver/Concussion Agreement.  Once you are logged-in, simply click the following link and fill in the required information to submit the Liability Waiver/Concussion Agreement for your child using our online system.
