News
For the 2024 - 2025 school year, we will be using an online system to complete most required paper work for our athletes.  The program is called Final Forms. Click on the Final Forms link below to register and complete the paperwork for your student - athlete.

Final Forms is an online system that will allow you to register your student - athlete and complete most required forms electronically, saving you time and less paperwork. The only form that is still required to be handed in is the actual OHSAA Physical Form that the doctor will need to sign.

2023-2024 Fall Sports Letter

General Information

As we quickly approach the start of the 2024-2025 school year, you will find information below that pertains to fall sports.

  • Your child must have a current Sports Physical Form in order to participate or try out for any sports for the school year. A copy of the physical can be uploaded into Final Forms (see below) or a copy can be given to the coach the day of tryouts. No exceptions will be made if a student does not have a physical as this is an OHSAA requirement.

    • A printable PDF copy of the Sports Physical Form can be accessed by clicking here.

  • Your child must be registered into Final Forms for the sport that he/she is participating or planning to participate in. This system is a communication tool used by the District to communicate upcoming activities and to track all student information such as physicals and payments for sports. The following links will take you to the page, as well as instructions on how to sign up for an account if you are new to the system.

  • The Pay to Participate fee rates this year have not changed for middle school athletics. Click here for a full list of Co-Curricular Activity Fees.  Athletic fees must be paid prior to the start of the season and all school fees must be current in order to try out or participate in a sport If fees are not paid, the student will not be able to participate in any contests until they are paid in full. Be sure to complete a Free and Reduced-Price Meals Form if qualified, as we refer to this document regarding sports participation fee rates. The Free and Reduced-Price Meal Application application can be filled out online by clicking here or submit a paper copy to our main office by clicking here.

  • Please do not pay fees for any sport (ie. Volleyball) that has team selection cuts until after cuts have been made and the team has been officially formed.

  • Fall Sports middle school participation fee rates:

    • Free Lunch: $40 per fall sport

    • Reduced Lunch: $65 per fall sport

    • Full Cost: $90 per fall sport

  • There will be a parent meeting for all Fall Sports TBD. This will be a virtual meeting and you will be able to ask any questions that you may have about the upcoming season.  A link to join the meeting will come soon.
