Please bring a copy of your child’s birth certificate and health insurance information

2012 Player Regisration
All Players Registering must fill out the Player Registration form and the Medical Release form, and the Parents must sign and bring the Parents conde of conduct form. Please bring these filled out forms with your proof of residency and your birth certificate to Registration.
