Football-Cheer Registration

 Tuesday, April 14th, 2015 – At HYAL Clubhouse from 7:00 to 9:00 PM

 Sunday, April 19th, 2015 – At Sports Authority from 12:00 to 2:00 PM

 Saturday, April 25th, 2015 – At Sports Authority from 12:00 to 2:00 PM

 

 You must bring 3 checks to registration with you. There will be a $25 fee for any returned checks.

 CHECK #1 REGISTRATION PAYABLE TO HYAL – Dated with the day you are registering.

 

 FOOTBALL Mitey Mites & above (7 and up)                      Flag Football (5 & 6 Year olds)

Cheer -Included in your fees are: Uniform, bodysuit, bloomers, practice tee shirt, and practice bow.

Please note that the Flag team and Mites team will be non-competition but will do an expo. Jr PW and older teams will compete.

 We can offer partial payment financing - If paying in installments, you would be required to leave 50% of your total amount due at registration, and then make payment on the remaining 50% by June 1st.

  

CHECK #2 PAYABLE TO HYAL - DATED - SEPTEMBER 1, 2015

Will not be cashed unless Work Duty Commitment is not fulfilled.

Work Duty Deposit $200.00 per family

 

CHECK #3 PAYABLE TO HYAL - DATED - OCTOBER 1, 2015

Will not be cashed unless Uniform & Equipment are not returned.

Uniform Deposit $200.00 per player/cheerleader

 

FORMS - All forms for registration can be obtained at our HYAL website www.hazlethawks.com. Click on either Football or Cheer tothe left and then Forms & Handouts. All participants must have the official AYF Physical Form filled out prior to 7/22/2015 or the player WILL NOT be allowed on the field. The Physical Form must be an ORIGINAL, it cannot be copied or faxed and must be signed, stamped & dated by your physician.

For First Time Registrants you will need to present a copy of your child’s birth certificate and produce a utility bill for proof of residency.
