Welcome to another season of Our Lady of Peace CYO Basketball. We will be conducting registration a little differently again this year. All forms for registration will be posted to our website as they were last year. We are asking all participants to download the forms and complete them before submitting them. All forms will be turned into the Rectory as indicated below.
Please go to the "Handouts: tab and you will find all the forms necessary to register for the 2011-2012 season. All forms are necessary to be completed prior to submitting them for registration.
A couple of forms that require special attention:
1. The QI and QII forms will need to be signed by the Director of Religious Education (DRE). If they are not signed by the DRE, they will be considered incomplete.
2. The Expected Behavior Form needs to be signed by both the participant and a parent.
We will not be accepting any incomplete paperwork at the time of registration. Registration dates will be Sunday September 11, through Friday, September 30.
There will be no formal sign-up this year. All forms and checks will be submitted by dropping the completed paperwork at the Our Lady of Peace Rectory. No paperwork will be accepted after September 30.
All those new to the program will need to submit a copy of their birth certificate as well. Anyone who participated in the 2010-2011 season, but did not submit a birth certificate will also need to submit a copy. Participation Fees will be the $130.00 for 5th through 8th graders and $75 for 4th graders. If you have more than one child in the 5th through 8th grade, each additional child will be $100.
