Registration dates for the 2015 season of football and cheer will be June 18-19, 2015 from 6pm-8pm and Saturday, June 20 from 9am-12pm in front of Smithfield High School. Late registration will be held Friday, July 31 from 6-8pm and August 1 from 10am-12pm. Cost is $125 for football and $100 for cheer. Teams fill up quickly so early registration is preferred.
Equipment needed by players: white helmet, white practice pants and shoulder pads. Parents must purchase. Not provided by Smithfield Pop Warner. 7 piece pads, practice jersey and game uniform will be supplied by Smithfield Pop Warner.
Documents needed at registration: player registration form, medical/physical form, birth certificate and final 2015 report card. Forms can be found in the handouts tab of website. Medical form must by signed by an MD, DO or NP.
