Wednesday June 6th at 5:30pm to 7:30pm
At Post 6 North Common Street
Football Fee: $125.00 per player Cheerleading Fee: $65.00 per cheerleader
Family Fee for two Siblings: $150.00 and $10.00 for each additional sibling
 
Girls and Boys ages 5-15 By August 1, 2012
 
You will need:  
 1. A West Lynn Application (will supply)
                       2. Birth Certificate (No Copies)
                       3. Pop Warner Medical Form Due August 1, 2012 signed by Doctor (in Application)
                       4. End of year report card will all grades front and back if copies
