WAIVER INFORMATION

I/We the parent(s)/legal guardian(s) of the above named child hereby give my/our full consent and approval for my/our child to participate in the Jr. Pro Stars Sports League.

I/We understand that there are certain risks of injury inherent in the practice, play, and participation in the leagues athletic programs, as well as in traveling and other related activities incidental to my/our child’s participation. I/We am/are willing to assume all of these risks on behalf of my/our child and I/we hereby certify that my/our child is fully capable of participating in the Jr. Pro Stars Sports League and in the league’s athletic programs. I/We also certify that my/our child is healthy and has no physical/mental disabilities/infirmities that would restrict or prevent full participation in these activities.

In addition to giving my/our full consent for my/our child’s participation, I/we do hereby forever waive, release, make a covenant not to sue, and hold harmless Jr. Pro Stars along with its officers, coaches, sponsors, staff, and representatives (including volunteers) for any injury that may be suffered by my/our child in the normal course of participation in the Jr. Pro Stars Sports League, its athletic programs, and the activities incidental thereto whether the result of negligence or any other cause.
