AUTHORIZATION (Please Read Carefully)
I hereby authorize the Stillwater Recreation League and its authorized coaches or league officers to obtain all necessary medical care for my child in the event of an emergency, and hereby authorize any licensed physician and/or medical personnel to render all necessary medical treatment to my child.
I UNDERSTAND that the registration fee is paid to partially cover necessary costs. No child is entitled to keep or destroy any league property. Any cost incurred by the League via action or inaction by the child and/or appropriate guardian/parent can be billed in full and collection pursued to the full extent of the law.
I UNDERSTAND that the league purchases “secondary type “medical and liability insurance; personal family medical plan represent “primary medical “plan coverage for my child.
I hereby certify that my child is in good health and has my permission to participate in the Stillwater Basketball program.

STILLWATER RECREATION CODE OF CONDUCT
The essential elements of character building and ethics, in sports are embodied in the concept of sportsmanship and six core principles: Trustworthiness, Respect, Responsibility, Fairness, Carin, and Good Citizenship.
I agree to abide by the SRL rules and regulations and the following guidelines. If I fail to do so, I will be subject to disciplinary action that could include, verbal warning by a coach, official or league representative head, game suspension and even suspension from all future League activities.
Player: I hereby agree:

1. I will respect my coach, the players on my team and the teams we play, and the officials.
2. I will learn the rules of the game and policies of the league as they are taught to me.
3. I will not engage in any kind of unsportsman-like conduct with any official coach.
4. I will accept all decisions made by the referees.
5. I will refrain from any actions that may endanger the safety or comfort of the players, coaches, or other spectators.

By submitting this registration I attest that the code of conduct was reviewed by each player and legal guardian
