EUREKA JFL SCHOLARSHIP APPLICATION

Eureka JFL is committed to providing playing opportunities to players whose family might not otherwise be able to afford the fees associated with participating in our program. The number of scholarships available is limited so we need to gather information from all applicants to the scholarship program to best determine which families have the greatest need for assistance. Additionally, due to the limited number of scholarships, each family is eligible for only one scholarship regardless of the number of children in your family who participate in Eureka JFL. In order for us to accurately evaluate each applicant's qualifications, we must have all of the questions answered.

The Eureka JFL Board will be reviewing all of the applications and will be responsible for deciding which families will receive the scholarships. All of the information contained in this application will be kept confidential, and any family that receives a scholarship will remain anonymous.
Once all of the applications have been reviewed by the Board, Eureka JFL will inform you if your family will receive one of the available scholarships. All decision by the Board will be final. In order to maintain confidentiality, no information will be disclosed regarding how the Board arrived at its' decision regarding your application or any other application.
