WAIVER INFORMATION

1. NATURE OF ACTIVITY - Futsal

I, the Releaser, wish to participate in the following Activity which is taking place between October 2016 and May 2019. I acknowledge that I am of legal Age required to do so:

South Central Futsal League based in Winkler, MB - a Non-Profit Organization

2. WAIVER

I, the Releaser, hereby waive, release, and discharge the Releasee (who runs and/or operates this Activity) and his or her Heirs, legal Representatives, Administrators, and Executors for all Liability for or by Reason of any Damage, Loss or Injury (including Death) to myself or my Property which has been or may be sustained as a result of my Participation in the Activity, and notwithstanding that such Damage, Loss or Injury may have been caused solely or partly by the Releasee’s Negligence.

3. ACKNOWLEDGEMENT

I, the Releasor, hereby acknowledge the following:

i. I have read this Release thoroughly and I fully understand it.

ii. I am voluntarily executing this Release, and would not otherwise be permitted to participate in this Activity.

iii. This Release restricts me from suing or otherwise claiming legal Action against the Releasee, presently or at any future time, for Damage, Loss, or Injury that may occur to myself or my property as a result of participating in this Activity.

iv. I have been granted the Opportunity to seek independent legal Advice before executing this Release.

4. BINDING EFFECTS

This Release shall be binding on and inure to the Benefit of the Spouses, Heirs, Executors, Successors, legal Representatives, Administrators, and assigns of the Releaser and Releasee.

5. MEDICAL CONDITIONS

To the best of my knowledge, I, the Releaser, am in good Health and do not have any Illnesses, Ailments, physical or mental Disabilities that would limit or prevent my Participation in this Activity. I will provide a medical Examination and Clearance if required.

6. FINAL AGREEMENT

This Waiver and Release contains the entire Agreement between the two parties, and there are no further Provisions, either written or oral. The Terms of this Waiver and Release are contractual and not a mere recital.

7. GOVERNING LAW

This Waiver and Release is being delivered in, and shall be governed, construed, enforced, and interpreted by, through, and under, the Laws of the Province of Manitoba, excluding Conflict of Law Principles that would cause the application of Laws of another Jurisdiction.

9. PUBLICATION

I, the Releaser, fully understand and agree that my Name and Photo may be made public on Websites and/or Social Media Accounts in Association with South Central Futsal for entertainment, promotional and educational Purposes. *

10. UNDER THE AGE OF MAJORITY

Are you UNDER 18 Years of Age and want to participate in this Futsal League? If yes, please print this Form AND the Consent Form found in the Forms-Library and have your Parent/legal Guardian sign them both. Once signed, please submit it to the League Director either via Email or In-Person. You will not be allowed to participate without a signed Consent Form.

South Central Futsal and its Associates shall not be held liable, when Damages and/or Injuries (including Death) to Registrants who intentionally provide false Information (e.g. false Date of Birth) about themselves, occur.

All Games of Teams accepting Individuals named above shall be forfeited until a signed Consent Form is handed in or the Individual reaches the Age of Majority.
