Special Requests
**There is no guarantee that all special requests will be granted. This is at the discretion of the EBSA Board.
Examples would include petitioning a player up to the next age division or requesting players be on the same team, such as step siblings.

WAIVER INFORMATION
I, the undersigned parent/guardian of the participant, a minor child, do hereby authorize the Eaton Baseball/Softball Association and the representatives thereof, as agents for the undersigned, to consent to medical, surgical, and/or dental examination in case of emergency. I hereby authorize treatment and/or care of participant at any hospital.

CODE OF CONDUCT POLICY
Violation of any part of the code of conduct will result in removal from the park and the party/parties involved will not be allowed to return to the park for the remainder of the game. A second violation will result in suspension from all EBSA activities for the remainder of the season.

1. The progress of the game will not be disturbed by a fan's action. The
umpire has the authority to eject fans from the ball park, if necessary,
with the assistance of the Eaton Police Department.

2. I will not use profanity or any type of verbal abuse toward any
participant, coach, umpire, or league official.

3. I will abide by the decision of the umpire and/or league officials.

4. I will be respectful of the coaches, umpires, and players during and
after the game. If I have any concerns regarding my child or a
disagreement, I will contact an EBSA Board Member.

5. I will inform EBSA and my child's coach of any physical disabilities or
ailment that might affect the safety of my child and/or others.

6. I will encourage my child to display good sportsmanship at all times
win or lose.

I agree that I have read and understand all codes set forth by EBSA. I understand that a violation of any of the codes will result in removal, suspension, or termination of any participation in league activities.
