403 BALL HOCKEY LEAGUE WAIVER INFORMATION

EACH AND EVERY PLAYER IN THE 403 Ball Hockey League IS REQUIRED TO READ AND ACCEPT THIS DUCUMENT. BY ACCEPTIING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. REFUSAL TO ACCEPT THIS DOCUMENT WILL PREVENT YOUR PARTICIPATION IN THE 403 Ball Hockey League. PLEASE READ CAREFULLY.

ASSUMPTION OF RISKS

I recognize and acknowledge that participating in activities, without limitation, offered by or associated with the 403 Ball Hockey League exposes me to many inherent risks, dangers and hazards. I agree to assume and accept full risk of any and all personal injuries including damages or loss, which I may sustain as a result of participating in any and all activities associated with the 403 Ball Hockey League.


RELEASE OF LIABILTY WAIVER OF CLAIMS & INDEMNITY AGREEMENT

In consideration of the 403 Ball Hockey League permitting me to participate in its activities, permitting me the use of its equipment and permitting me the use of its facilities, I hereby agree as follows:

1. TO WAIVE AND RELINQUISH all claims I may have against the 403 Ball Hockey League and its officers, agents, servants, players, sponsors and anyone connected to the 403 Ball Hockey League.

2. TO RELEASE AND DISCHARGE the 403 Ball Hockey League and its officers, agents, servants, players, sponsors and anyone connected to the 403 Ball Hockey League, from any and all claims for injuries, including damage or loss which I may have or which I may accrue to me as a result of participation in the 403 Ball Hockey League.

3. TO INDEMNIFY AND HOLD HARMLESS and defend the 403 Ball Hockey League and its officers, agents, servants, players, sponsors and anyone connected to the 403 Ball Hockey League from any and all claims resulting from injuries, including damages and losses sustained by me and arising out of, connected with, or in any way associated with the activities of the 403 Ball Hockey League.

4. That this Agreement shall be effecting and binding upon any heirs, next of kin, executors, administrators and assigns in the event of my death.


I UNDERSTAND THAT THIS ABSOLUTE RELEASE OF LIABILITY, EVEN FOR THEIR OWN NEGLIGENCE, IS AN INTEGRAL REQUIREMENT AND PRE-CONDITION OF PARTICIPATION IN THE 403 Ball Hockey League. I, THE PLAYER (OR LEGAL GUARDIAN), HAVE CAREFULLY READ THIS WAIVER, AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS AND I FREELY AND VOLUNTARILY ACCEPT ITS TERMS.

CHECKING OFF THE “I ACCEPT’ BUTTON BELOW SERVES AS PROOF THAT I HAVE READ AND ACCEPTED THE TERMS AND CONDITIONS OUTLINED HEREIN.
