In order to be considered for a coaching position for the 2021 season, I give permission for STREATOR GIRLS SOFTBALL (SGS) to conduct a background check on me. My coaching position is conditional upon the league receiving a clear background check.

I am attesting that I will NOT hold any SPONSOR of any team or the STREATOR GIRLS SOFTBALL League responsible for any accidents or injuries that might occur during the softball season.
