PLEASE READ AND AGREE TO THE FOLLOWING:

I agree to complete the SUMMER HOCKEY LEAGUE PROGRAM AGREEMENT AND RELEASE FROM LIABILITY FORM. Failure to do so will prohibit my son and/or daughter from stepping onto the ice. I understand that this Liability form MUST be mailed in with payment.

I agree that all games will be on Sunday's and/or Monday's.

I agree to have my son and/or daughter's full or partial name on the SHL website that is password protected.

I am responsible for my own water bottle and putting my name on it.

I will be following HEO's Rules & Regulations, including their Code of Discipline, with the exception of the following SHL rules:
- Games will consist of a 2 minute warm-up and 45 minutes of running time.
- Players who receive a total of 9 minutes in penalties or more will be removed for the remainder of game.
- Minor penalties will be 3 minutes long.
- Major penalties will be 7 minutes and a automatic game misconduct.
- Players receiving a game misconduct WILL NOT protest and will remove themselves from the playing area. The SHL will notify the player(s) via email on number of games to be served.
- Players receiving a penalty for Unsportsmanlike Misconduct, will sit in the penalty box for the amount of time given OR at the discretion of the referee, a game misconduct!
- Players receiving a Fighting major will automatically receive a Game Misconduct and the VP will notify the players via email on the number of games they will be suspended for.

I agree that I have to pay $100 if my son and/or daughter's looses their SHL jersey.

I agree that the Summer Hockey League is Non-Contact.

I agree that once I pay there will be NO REFUNDS!
