In registering for the 8 Wing/CFB Trenton Sports program, I am aware of the following::

I may be suspended for an indefinite period of time due to my actions in this NPP Sports League; *

I am not considered on duty when participating. Coverage by the employer or WSIB or any other such program is not guaranteed as it is not a normal place of work and aware there are inherent risks associated with participating; *

I am a Veteran or full time employee of DND or NPF, at 8 Wing/CFB Trenton; *

I have approval from my respective supervisors for participating; *

I am not currently serving a sport suspension in the CF or 8 Wing/CFB Trenton Sports Program; *

I do not have any health problems that may preclude my participation; *

I am aware to pay $75 for Veterans and $100 for NPF/DND (per league- WHL/NHL) per year and there will be no reimbursement; *

I acknowledge that I have read "Waiver of Liability, Assumption of Risks, and Indemnification Agreement" and assume all risks and liability associated with the Agreement. *
