MEDICAL CONSENT & RELEASE LIABILITY AGREEMENT - Agreed upon by signing up your child.

I hereby give permission for my child to participate in the 2026 All Saints Parish and Butler Catholic School Champions of Faith Basketball Camp. I declare that that I am the parent or legal guardian of the above named child, and I have custody and control of the child. In the event my child is injured or should require medical attention, I hereby request that you contact me or our emergency contact. In the event that we cannot be reached, I hereby authorize All Saints Parish and Butler Catholic School to secure necessary medical treatment for my child. I further acknowledge that I will be responsible for any medical or hospital fees or costs associated with my child's medical treatment. I understand that as a participant, my child may be participating in rigorous activity. I assume all risks and hazards to such participation, including transportation to and from camp, and hereby waive, release, absolve, indemnify, and agree to hold harmless All Saints Parish, Butler Catholic School, organizers, volunteers, and participants for any claim arising out of accidental injury to my child. My signed name below confirms that I have read, understand, and agree to the terms of this Release from Liability.

As a reminder, Little Dribblers is for rising 1st - 3rd graders, Offensive Skills and 3on3 is for rising 4th-6th graders.

If you have multiple children participating, your oldest child pays full price, and each additional child is eligible for the Sibling Discount for their camp type. For example, my 5th grade daughter is participating both the 3 on 3 and Offensive skills camp, so I would register her at the $100 fee. My 2nd grade son would then need registered for the Sibling Discount - Little dribblers camp.
