MEDICAL/EMERGENCY CONTACT INFORMATION
We will allow 5-6 year olds to play in either the Coach Pitch or Pitching Machine league. Please understand that if you choose to have your child "play up", that games and practices are much more fast-paced and there is always a greater risk in their being injured on the playing field due to the fact that they will be playing with children as old as 8.
Please indicate which age bracket your child wishes to play in.


WAIVER INFORMATION
I/We, the parent(s) of the above named candidate on a sports team, hereby give my/our approval to participate in any Carlisle County Sports League activity, including transportation to and from activities as needed. I/We recognize that participation in sports may result in serious injuries and that protective equipment does not prevent all injuries to players. I/We do hereby waive, release, absolve and agree to hold harmless the Carlisle County Sports League and its coaches and volunteers. I/We agree once your child is placed on a team, there will be no switching or moving teams once teams are placed. I/We agree that the above named candidate will play at the league level assigned to him/her based upon the guidelines.
