2013 Fall Individual Player Waiver & Release

*Must be 21 years of age to be able to participate in the SKA League.

LIABILITY DISCLAIMER
Please read carefully and INITIAL the box below to accept the Terms of the Amateur Athletic Waiver and Release of Liability Form.

In consideration of being allowed to participate in any way in Saint Louis Kickball Association, LLC, programs and related activities,

A. Agree that prior to participating, will inspect the grounds and equipment to be used and if I believe anything is unsafe, will immediately advise my coach or supervisor of such condition(s) and refuse to participate.

B. Acknowledge and fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability and death and severe social and economic losses, which might result from, not only my actions, inactions or negligence of others, but from the rules of play, the condition of the premises or any equipment used. Further, that there may be other risks not known to me or not reasonably foreseeable at this time.

C. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.

D. Along with my family, spouse and my heirs, release, waive, discharge, and covenant not to sue Saint Louis Kickball Association, LLC, it’s affiliated clubs, their respective administrators, directors and other members of the organization, other participants, team coaches, sponsoring agencies, sponsors, advertisers and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releasees” from demands, losses or damages on account of injury, including death, or damage of property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.

E. Am of good physical condition to play kickball in an ever-changing outdoor setting on natural grass and/or dirt.

* The SKA Cup, awarded to the team that wins the tournament at the end of the season, is the property of the Saint Louis Kickball Association, LLC. By signing this Individual Player Waiver & Release Form, the captain of the team, whose name appears at the top of this form, acknowledges that if said team wins the tournament and is awarded the SKA Cup, will return the trophy, in as good a condition as when it was received, before the start of the season the following year. If the trophy is not returned, the Saint Louis Kickball Association, LLC, will take whatever legal means possible to ensure the trophy is returned.
