WAIVER INFORMATION

The undersigned hereby grants permission for the child noted above as "PLAYER" to participate in the athletic program sponsored by the WCIL EXPRESS Baseball. It is understood that WCIL EXPRESS Baseballis a non-profit volunteer organization which sponsors the above noted sports program as a community service and ministry outreach. WCIL EXPRESS Baseball is not responsible for the expenses or damages resulting from injuries to participants or spectators which are sustained in conjunction with or incidental to the above activity. Accordingly, the undersigned, HEREBY EXPRESSLY AGREES to waive all claims against and hold exempt from liability WCIL EXPRESS Baseball, its officers, directors, coaches, and any other person affiliated with WCIL EXPRESS Baseball in an official capacity, for injury or injuries sustained by the PLAYER, from whatever cause, while attending, participating in, or traveling to and from WCIL EXPRESS Baseball activities. It is FURTHER AGREED that the undersigned hereby waives any and all claims against Victory Baptist Church, it's people or pastor, or any sports league in which WCIL EXPRESS Baseball participates and their directors, officers, supervisors, managers, or any other person affiliated with these organizations, for injury or injuries sustained by the PLAYER, from whatever cause, while attending, participating in, or traveling to and from WCIL EXPRESS Baseball activities. It is further understood and agreed by the undersigned that WCIL EXPRESS Baseball is a faith-based Christian organization that will, from time to time, in addition to and in the course of athletic and sporting activities, engage in Christian religious activities and teaching, and that the Player may be introduced, exposed or subjected to such activities through their involvement in the WCIL EXPRESS Baseball program.
