2020 Registration

PLAYER INFORMATION
**T-Ball Boys & girls** Must turn 4 by September 1, 2020. If player meets age for Junior PeeWee, he/she must play in that division.

**BOYS BASEBALL AND CO-ED JUNIOR PEEWEE** Boys at all levels of play & girls Junior PeeWee - age determined as of May 31, 2020

WAIVER INFORMATION
Please read this form carefully and understand that by registering your minor child/ward for participation in the Rome Sullivan Youth Baseball program (hereafter known as “RSYBA”), you will waive and release all claims for injuries you or your child/ward might sustain arising out of the program.
RISK OF INJURY: “I recognize and acknowledge that there are certain risks of physical injury to participants in the RSYBA and I agree to assume the full risk of any such injuries, damages or losses regardless of severity which I or my child/ward may sustain as a result of participating in any activities connected or associated with the RSYBA.” “I do hereby fully release and discharge the RSYBA Board of Directors, coaches, and volunteer staff from any and all claims resulting from injuries, damages, and losses sustained by me or by my child/ward, and arising out of, connected with, or in any way associated with the activities of any of the RSYBA.”
MEDICAL ASSISTANCE: “In my absence, I authorize the coaches or other RSYBA personnel to use their judgment concerning medical care in case of emergency, illness, or injury of my child/ward during RSYBA activities.”
UNIFORM/EQUIPMENT: “I agree to return upon request, any equipment and/or supplies issued. I will return these items in as good condition as when it was received (except for normal wear and tear) or I will pay to replace the item(s). I understand that failure to return any portion of the items and/or equipment will result in a reasonable reimbursement fee.”

CONCUSSION ACKNOWLEDGEMENT
By typing your name below I have read the Parent Concussion and Head Injury Information that can be found in the All Printable Forms Menu Tab and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected.
I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me.
I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach.
I understand the possible consequences of my child returning to practice/play too soon.

CONCESSIONS ACKNOWLEDGMENT
RSYBA is able to keep registration fees low in part by thanks to the efforts of all the parents in supporting our concession sales. In an effort to “share the load”, by signing below, you agree to participate by assisting in the concession stand during 2 home games of your choosing. After Registration is completed, we will send out an email informing everyone about how to sign up for working this year’s concession. This year you can choose to work during your child's game or any other game that fits your schedule. Signups will be on a first come, first serve basis. If working in the concession stand will not work for your family, there are also options to assist with field prep, empty park garbage cans into the dumpster or assist in cleaning the park bathrooms. After each shift worked, you will receive $10 back of your concessions fee. Thank you for your support!




In an effort to help make concession's run smoothly, we have continued with a $20 fee again with this years registration. You will have to option to reclaim your $20 by working in the concession stand as described in the Concession Acknowledgment section of this form.
