MEDICAL/EMERGENCY CONTACT INFORMATION

We agree to allow our player listed above to tryout/play for the Warbirds, the year 2017 season. We understand that the Warbirds Baseball will provide the required insurance needed to practice/play on private fields/facilities. We agree not to hold the Warbirds Baseball, the owner, it's coaches, players, or administrators liable for any other injuries or damage that may occur as a result of playing with this organization.
