WAIVER INFORMATION

We, the parent(s)/legal guardian(s) of the player registered on this form, give permission for her to participate in the New York Inferno Fastpitch Travel Softball Program as a player and agree to hold the New York Inferno Fastpitch Travel Softball Club, Directors, Coaches, Instructors and any related sponsors harmless for any loss/damage resulting from her participation in this program.

ACCEPTANCE

Upon acceptance to a New York Inferno team, We/I, the parent(s)/legal guardian(s) further understand that the team may play as many as 7-8 tournaments of which 3-4 may be out of town and require an overnight stay (accommodations and transportation are an additional investment by each family and are not included below).

We/I also understand that the $450.00 cost of membership to use the facility at Athlete Factory for off-season workouts is also not included below and must be handled directly with the staff at Athlete Factory by each individual family.

Team practices will include an off-season schedule running approximately from October thru March. Day and time for these workouts will be determined in the near future. Practices are an important part of the program and every effort should be made to attend all practice sessions. Attendance will be kept and monitored.

The playing time of each player will be determined throughout the year by the coaching staff and will vary from player to player. There are no guarantees.

Playing time will depend on ability, attitude, effort, and attendance at team practices, off-season workouts and games.

Each family will be asked to contribute to the success of the team in some way in addition to the financial commitment shown below.

Player Fees will be determined using the following criteria:
* League play and Insurance
* Umpires
* Tournament entry fees
* Uniforms, team apparel, miscellaneous team equipment, etc.
* Field Use/Maintenance
* Website Development

This registration form, copy of birth certificate and photo ID (if required) and a $350.00 NON-REFUNDABLE deposit ($350.00 are due upon acceptance to a New York Inferno team. You must be prepared to make the required acceptance deposit at tryout.
