I, by signing below, as parent/guardian of the above named participant, hereby give approval for his/her participation in any and all Jennings County Boys Basketball Camp events. I am aware that, as in any sport, there is a risk of injury, which comes from participation in an athletic activity. I assume all risk and responsibilities in case of accident while this child is participating in this camp. By allowing the above participant to participate in this activity, I agree that Jennings County Schools, its employees and associates, the Jennings County Boys Basketball program, and/or any other participants will not be liable for medical, dental, or other expenses incurred as a result of accidents or injuries sustained while participating in this activity.
