Player Registration Form

I, as parent/guardian of the above-named player, hereby give my permission to participate in the Adams Lassie League. On behalf of such player, I assume all risks and hazards to such participation, and hereby waive, release, absolve and agree to hold harmless, The Adams Lassie League, its officers, volunteers, agents and related parties (hereinafter collectively called “the League”) for any and all claims arising out of any injury to such player; including, without limitation, any claim for personal injuries resulting from or arising out of the negligence of the League in connection with participation in the League. On behalf of such player, I assume all responsibility and certify that such player is in good physical health and is capable of participation in the League.

PLEASE NOTE:
The physical nature of softball involves inherent risks and hazards of physical injury. The League is not responsible for injuries suffered by players. If a player wishes to avoid the risks and hazards inherent in softball, the player SHOULD NOT PARTICIPATE. The League is not responsible for injuries suffered by players.



I give the Adams Lassie League permission to take and publish photos of my player on social media the league web-site & local media outlets
Parent Code of Conduct

I hereby pledge that our family will support our Softball team by adhering to the Parent Code of Conduct as stated below:
• We will let the coach be the coach and will respect the coach’s decisions
• We will show interest, enthusiasm, and support for all children on the team
• We will provide only supportive comments to coaches and players of both teams. We will avoid any derogatory comments. If we have concerns to bring to the coach’s attention, we will apply the “24 Hour Rule” before having a respectful, private conversation
• We will not criticize or question umpires’ calls. We will let the umpires be the umpires.
• We will not coach our daughter during the game.
• We will avoid the use of alcohol and tobacco products at practices or games or come having drank too much.
• We will remain in the spectator area and we will be in control of our emotions.
• We will help when asked by coaches or umpires.
• We will help enforce this code through appropriate means.
The Adams Lassie League and The Greylock Thunder Travel League have enacted this code for all to follow. Failure to adhere to these rules of conduct could result in temporary and/or permanent suspension of Game privileges.
Player’s Code of Conduct

I pledge to honor and support my team by adhering to the team’s Player Code of Conduct as stated below:
I will treat all players, coaches, umpires, parents, and spectators with dignity and respect, as I would like to be treated, using appropriate language in appropriate tones when interacting with them.
I will arrive on time for practices, meetings, and games with only my own religious obligation, illness, family emergency, or injury being acceptable reasons for tardiness or absence.
I will seek to become the best athlete I can be by practicing appropriately, trying my hardest on the field, and concentrating on the play of the game. I will learn to take correction as a compliment.
I will win and lose graciously, demonstrating good sportsmanship throughout games and practices.
I will be honest and take responsibility for my actions and results.
I will play for the team, not myself.
I will encourage and assist my teammates, giving encouragement and support in success and struggle.
I will adhere to the rules of the sport and the policies of the Adams Lassie League (attendance policy)
I have read and understand the above statements and agree to conduct myself in a manner consistent with each
