Registration Fee Waiver Form

Adams Lassie League firmly believes that each child who wishes to participate in softball are entitled to do so regardless of skill or financial circumstances

WAIVER ELIGIBILITY REQUIREMENTS

To be eligible for a registration fee waiver, you must provide the following:
• An explanation of the financial hardship (please explain in the FINANCIAL HARDSHIP EXPLANATION section) – supporting documents may also be requested
• Proof of residency – indicating that the player(s) reside(s) in Adams, Cheshire or Savoy or Surrounding area.
• Proof of age - - indicating that the player(s) meet(s) Adams Lassie League requirements to play in the league. All registration paperwork must be completed in full.
