Please READ:

1.By signing below, I am attesting that I will NOT hold any SPONSOR of any team or the STREATOR GIRLS SOFTBALL League responsible for any accidents or injuries that might occur during the softball season.

2. I agree to one of the registration fee options for each registered player.
A. I will sell 10 raffle tickets at $10.00 each OR
B. I pay $100.00 and sell NO tickets.

3. I, the parent/guardian, am responsible to pay for any UNSOLD tickets.
(PLAYERS WILL NOT RECEIVE THEIR UNIFORM SHIRT NOR BE ALLOWED TO PLAY UNTIL ALL MONIES ARE TURNED IN.)

**If you sold ALL 10 of your tickets for the 2020 season, you will NOT have to sell tickets for the 2021 season, unless you would like to.
