WAIVER INFORMATION
I give permission for my child to participate in Fort Cherry Youth Fast Pitch. I will not hold the organization and sponsors, their employees and associated personnel, including the owner of fields and facilities utilized for the programs against any claim by or on behalf of the registrant as a result of the registrant’s participating in the programs and/or being transported to or from the same, which transportation I hereby authorize.
I also understand that Fort Cherry Youth Fast Pitch carries secondary insurance only and that it is limited to injury coverage only, and that no one associated with Fort Cherry Youth Fast Pitch will be responsible for damages to any personal property.
I have full authority and do hereby authorize the Fort Cherry Youth Fast Pitch, its officers, directors, members, and/or agents to display the image and/or likeness of my child on any website authorized by Fort Cherry Youth Fast Pitch. I shall not receive any compensation for such use. This Authorization shall remain in effect until revoked by me or my child in writing.

NEW REGISTRATION REQUIRMENTS
FCYFP has implemented new registration rules and procedures. You must check each box below, acknowledging you have read and understand these requirements.
Spring Registration Fee ... $95.00 / $50 for 2nd child

Spring Fundraising Deposit ... $75 check per player to be cashed only if fundraising goal for the season is not met (see website for details)
*
Team rosters will be limited to no more than 13 players per team
*
Registration must be finalized in person at Fort Cherry Elementary Center - either Tue, Jan 28 from 6:30 PM - 7:30 PM or Sat, Feb 1 from 12:00 PM - 1:00 PM
*
Roster eligibility is contingent upon FCYFP having a complete and finalized player registration - this includes receipt of registration form, birth certificate, signed code of conduct, and payment
*
Team rosters will be filled first-come-first-served based upon roster eligibility list with priority given to Fort Cherry School District residents
*
There is a league limit of two out-of-district players per team
