2020 HS ELITE TRAVEL BALL



You have the right to workout/tryout/practice in or at a safe, clean, and welcoming environment. Social distancing will be used whenever possible. If, at any time, you feel uncomfortable, please let a coach know. To prevent the spread of COVID-19 and reduce the potential risk of exposure, your honest answers to this portion is crucial! One parent or legal guardian will be allowed to attend this tryout, however, future attendance will be modified based on the coaching staff, location, venue, and protocol requirements. TO REDUCE RESPIRATORY DROPLETS, DO NOT SCREAM, YELL, CHEER, OR COACH FROM THE SIDELINE.

RELEASE & WAIVER
I/we hereby attest that I/we are fully cognizant of the assumed risks involved in the sport of basketball and the Prodigy Athletic events. I/we voluntarily assume and agree to abide by the published and spoken rules of this organization and protocols, at all times, regardless of time and location, and for the entire length of student-athlete's participation.

GOOD HEALTH
I/we acknowledge and fully understand that my/our student-athlete will be engaging in a contact sport that might result in serious injury or death, due to or not due to my/our student-athlete's actions. I/we further attest that the student-athlete does not suffer from any known heart condition, is in good health, and can handle the contact associated with the sport. In the event of any injury, our certified trainer or a qualified staff member will provide immediate examination and/or treatment. If I/we cannot be contacted, the emergency contact will be called, and/or be directed to transport the student-athlete by car or ambulance to the nearest medical facility (CED section 11710).

ASSUMPTION OF RISK
I/we assume all liabilities, and release, waiver, discharge the right to pursue and legal (past,present,or future) against the organization, administrators, school districts, schools, school staff, employees, organizers, volunteers, coaches, incidental or direct liability connected with the student-athlete's participation.

TEAM PLACEMENT & PARENTAL CONTROL
Team placement is based on the information provided along with your TRYOUT.
I/we understand that this is a competitive yet off-season/recreational athletic event. As adults (parents/spectators) will respect the rules of the game, game site, staff members, volunteers, and administrators. Any misconduct or violation of any COVID-19 protocols will be handled immediately, and may result in complete dismissal.

LEGAL
Prodigy Athletic Institute is a legally insured and nationally recognized institution. The use of our legal name (Prodigy Athletic Institute and in any form) and/or reproduction of that name is strictly prohibited. I/we fully authorize the use of my/our student-athlete's name, likeness or image for the purpose of advertising, marketing, promotion of the program without any compensation. Information, photos, videos, stats, and updates are posted on Facebook (Prodigy Athletic Institute), Instagram (@prodigyathlinst), Twitter (@prodigyathlinst), and our website (www.prodigyathletic.com). College entrance information will only be shared by our staff, by requests only, by college coaches. We will not distribute your information to recruiting services or spokespersons.

PAYMENT
PRODIGY ATHLETIC ADMINISTRATIVE COSTS
There are no refunds, credits, or rollovers regardless of time, situation, conditions, or control including, but not limited to Acts of God, Government restrictions, wars, insurrections, natural disasters, such as earthquakes, hurricanes or floods and/or any other cause beyond the reasonable control of the owner.

OUTSIDE EVENTS
Refunds from the cancellation of outside events out of our control (tournaments, hotels, gyms, and/or transportation) is at the discretion of the executive staff.

NCAA CERTIFICATION
As a member of the annual NCAA certification system, the Prodigy Athletic Institute staff is required to report accurate student-athlete information and attend NCAA certification events. With these responsibilities, we certify to an open and honest participation process, in addition to healthy partnerships with college coaches and their staff as well. With these agreements comes the responsibility of accurate vetting through the recruiting process (academics/transcripts, attitude, aptitude, personal development, character, etc). We urge every student-athlete to adjust all social media accounts. VIOLATING NCAA RULES COULD RESULT IS DISQUALIFICATION.
