IMPACT & NBN BASKETBALL COVID WAIVER

Participation Consent Form

Parents and players understand that the below are merely per cautions we will take to try and avoid any COVID-19 concerns. If you have any questions Parents and players should review any CDC and/or state requirements.

If your child has a temperature greater than 100.4 degrees or is showing any symptoms consistent with COVID-19, the child will not be allowed to participate. The student will need to be evaluated by a physician and if they test positive for COVID-19 they must be fever free for 24 hours without fever reducing medications and improved symptoms and 10 days after onset of symptoms prior to returning.

All spectators must wear a mask.

Parents and players are responsible for the washing and sanitizing of any gear before and after tournament.

Each player will use hand sanitizer upon arrival prior to participation. Hand
sanitizer is provided.

Each player should have a personal water bottle. Players should not share water bottles.

Players will immediately report any illness to their coach.

By signing this paper, I acknowledge that I have read and understand my role and responsibilities set forth by NOTHING BUT NET'S Play/Participate Guidelines.

(This agreement will stay in effect until conditions deem unneccessary).
