WAIVER INFORMATION
By checking the box below, I agree to not hold the Maine Basketball League, the Gilman Street Basketball Club, the Alfond Youth and Community Center, or its staff and administrators accountable for any mental or bodily injury that may occur as a result of participating in the league, including the risk of contracting COVID-19. I acknowledge that poor behavior resulting in removal from the league ensures that no refund (full or partial) will be granted. I also agree to abide by all MBL, GSBC, and AYCC rules while participating in the league and while on the GSBC or AYCC premises. I also agree to allow the league to share photos and videos of myself during and while participating in the league. I also agree to sign a new COVID-19 waiver form from the AYCC before participating in the league.

I agree to pay my player fee ($125) on or before 3/24/2021 to secure my spot. Payments can be accepted via Facebook or Venmo (@raymond-bernier). Payments not made on or before the draft day will will result in removal from the league draft.
