CONSENT AND AGREEMENT- I/We understand that the insurance, which is carried by us/own person is our primary insurance. In the event of a claim, I/we agree to submit the claim to our insurance company. I/We will not hold Kentucky Youth Football League liable for any injuries. I/We as parents understand the policy set forth by Kentucky Youth Football League that only coaches with badges and Guest Professional training coaches are allowed on field with the campers. All others must remain in the spectators area. I/We, hereby, for myself/ourselves, our heirs, executors and administrators waive and release any and all rights and claims for damages or injury against Kentucky Youth Football League, Inc. and their board members, guest trainers and coaches for any and all injuries suffered by our child during the football camp. I also understand that all payments are final and refunds will only be given if the event is cancelled by the Kentucky Youth Football League.
