WAIVER AND REFUND INFORMATION
I/We, the parents/guardians of the registered player for a position on The Long Island Elite Baseball team, hereby give my/our approval to participate in any and all activities. Agree that I/We will inspect the facility and equipment to be used, and if you believe anything is unsafe, I/We will immediately advise your coach of such conditions and refuse to participate. Acknowledge and fully understand that you will be engaging in activities that involve risk of serious injury, including permanent disability or death, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or any equipment used. Further, that there may be other risks not known to us or not foreseeable at this time. I/we assume all the foregoing risk and accept personal responsibility for the damages following such an injury, permanent disability or death. Release, waive, discharge and covenant not to sue the Long Island Elite Baseball LLC., their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releasers”, from any kind and all liability to each of the undersigned, his or her heirs and next of kin and all claims, demands, losses, or damages on account to injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasers or otherwise. The Long Island Elite Baseball LLC. may use baseball related photographs/videos of the participant on the website for marketing purposes.

REFUND POLICY; Our policy is a simple one, we do not offer any refunds at all, Seasonal deposits/payments are moral and financial commitments by the player and their families. There are no refunds on player injuries at any time, injuries are part of all sporting events and unfortunate for the player and his/her team. There are no refunds on players playing time or lack thereof, unhappiness with his/her spot in batting order. There is no refund if player/parent is asked to leave team per ' Elite Code of Conduct ' violations.. This is explained in your Code of Conduct that you have signed and submitted to the Organization.

By submitting this application you agree with the above and understand the PARENT/PLAYER CODE OF CONDUCT and PLAYER WAIVER/REFUND POLICY and have given up substantial rights.
