MEDICAL/EMERGENCY CONTACT INFORMATION

CARLISLE DIAMONDS MEDICAL WAIVER INFORMATION
Fastpitch softball is an exciting sport which may involve collisions with other players, the ball, or the ground. The sport is often played in hot, humid weather. Due to these conditions, which are part of the game, players are exposed to the risk of serious injury. Injuries could include, but are not limited to, broken bones, concussions, paralysis, damage to internal organs and even death. Such injuries can result in short term loss of function and/or long term impairment of physical abilities. In an effort to make the game of softball as safe as possible, the coaches of the team will teach the players the skills and rules of softball. Players must follow the coaches’ instruction, rules, and policies to reduce the possibility of injury. Team rules and polices are distributed to players and families at a pre-season meeting.

I/we, the undersigned, have read the warnings above and understand that fast pitch softball is a physical sport and that there are risks involved in participation. I/we further understand that there is a possibility that my/our daughter could be injured as a result of her participation.

I/we understand that the Carlisle Diamonds Organization carries team medical insurance to cover players who are members of the team on an “excess” basis only, and that my/our personal insurance will be utilized first.

I/we hereby, on behalf of my/our child and for myself/us, my/our child’s heirs, executors, and administrators do waive, release, and forever discharge any and all rights and claims for damages which I/we or my/our child may have or which may hereafter accrue me/us or my child against the Carlisle Diamonds Organization and its respective officers, representatives, successors, and coaches for any injury incurred during practice, games, or supervised team travel to and from same; and by signing where designated below, acknowledge that I/we, as parent(s) or guardian (s), has/have received, read, fully understand and agree to all the terms and conditions of this waiver.

I/we give my/our permission for the above named girl to participate on the Carlisle Diamonds Organization for the 2022 Spring season and I/we hereby certify that she is physically fit to take part in the softball program. I/we further authorize the coaches, or a representative of the Carlisle Diamonds Organization, to use his/her best judgment to protect, assist, and seek medical attention for the above named minor in the event of an accident or injury.

The purpose of this agreement is to establish a basic understanding of the expectations, rules and responsibilities between the parent(s) and the Carlisle Diamonds, Inc. & its coaches.

Philosophy: The purpose of the Carlisle Diamonds is to provide a venue in which to encourage, develop and support girls fastpitch softball teams. Your child will receive instruction on an individual and team level that will make her a more successful softball player. We will do the best we can to assure that happens. We ask that you give your best effort to support your daughter and her team.

Parental Responsibilities:
• All players are expected to attend all practices and games
• Players who are consistently late to practice or late to the reporting time for a game will not start, but will remain eligible to play
• Players who miss a practice or a game without notifying the coach could lose playing time in the next game.

** Exceptions to the above include illness, medical issues, school sponsored or related events, and family emergencies. In each case, players should notify coaches at least 24 hours in advance, when possible, but must directly contact a coach prior to practice or game time

Playing time: Playing time is regulated by league rules and the discretion of the coaches. Players’ time on the field is determined by the coaches based on objective evaluations of a players skills and the team needs on a game to game basis.

Communication: If you or your daughter have a specific issue or question related to her performance, a conference with the coach can be arranged. This conference will NOT occur just prior to or immediately after a game. In no circumstances, will the status of any other player be discussed. If you are not satisfied with conference, you can notify the Carlisle Diamonds Secretary, Jenn Karhnak, at carlislediamonds@gmail.com. She will pass the info along to the appropriate party.

Parent/Spectator conduct: The Diamonds gladly welcome the support and enthusiasm of all who will positively impact our teams. We expect our team members and our fans to always conduct themselves in a manner which brings honor and a positive light to our organization. As well, unless invited by the coaching staff, family members should remain outside the dugout area so players can focus on coaches' instructions during the practice or game. Please let our coaches coach and the umpires umpire.

Financial:
Fundraising: In order to keep our fees reasonable, we may engage in various fundraising projects. Please support our fundraising efforts. If you have specific fundraising ideas, please let one of the Carlisle Diamonds Board members know.

Know the Game: Girls softball is not boys baseball. Softball has its own unique rules and strategies. There are varying philosophies on how to bat, when to steal a base, how to pitch, etc. If you would like to learn more about fastpitch softball and the rules associated with the game, please visit the ASA website at www.asasoftball.com.

CARLISLE DIAMONDS PLAYER CONTRACT
The purpose of this agreement is to establish a basic understanding of the expectations, rules and responsibilities between the player and the Carlisle Diamonds, Inc. & its coaches.

Players: We expect that you come to the Diamonds with a desire and commitment to learn and play softball. Always bring a positive, “coachable” attitude. Be prepared to give your best effort in practices and games. Always remember that your actions, on and off the field, reflect on yourself, your teammates, your coaches, and your families.

• Come to practice prepared to PRACTICE, LEARN, and have FUN
• HUSTLE at all times
• Maintain a positive attitude and be supportive of your team throughout practice
• Bring all of your equipment to every practice

Game Extras:
• Have your cleats on and ready to go at the assigned warm up time for each game. This will generally be 30-45 minutes prior to the start of the game unless otherwise indicated by the head coach.
• During the game, please sit or stand with teammates in the dugout or bench area and stay focused on the game. You should not leave the dugout area without direction from the coaches. There should be no contact with your family during the game unless permission is granted by a coach

• Leadership is expected from every player. Support of your teammates, taking charge of equipment, getting warm-ups started, and pumping up your team are all ways that you can exhibit leadership. You are the leader – don’t wait for someone else to take charge when you know what to do!
• When a coach corrects you or tells you about a mistake you may have made, understand that this is coaching – not criticism of you as a person. Our goal as coaches is to make you a better player and help you to correct problems. If you don’t understand something, ask questions!
