Indian Creek Chiefs Youth Football Registration Form

1. Players must be present to get fitted for helmet and uniforms.
2. Parents must fill out required league paperwork. 1st year players
must provide original birth certificate for OVYFL to approve.
3. Registration limited to 30 players per division.
4. Returning players take preference over 1st year players.
5. Online Payment ONLY.
6. Offline Payment option for Financial Assistance Only.
Weight limits:
JV Division 130lbs
Varsity Division 180lbs

The Indian Creek Chiefs will not disclose personal information, including Email, address and phone information to any other party, except as required by law.

* Per OVYFL guidelines, all first year player registrations require an original birth certificate for proof of age.
I WILL PAY THE REGISTRATION FEE AND SIGN UP TO WORK 1 HOME GAME (OR PAY $25 INSTEAD OF WORKING HOME GAME)

I WILL BE RESPONSIBLE FOR ALL MONIES FOR THE REQUIRED FUNDRAISERS THAT MY CHILD IS REQUIRED TO SELL!

I, AS THE RESPONSIBLE PARTY, ASSUME LIABLITY FOR THE RETURN OF ALL EQUIPMENT ISSUED BY THE IC CHIEFS, OR BE LIABLE FOR THE REPLACEMENT COSTS.

I, the parent or legal guardian of the above registered child, fully acknowledge that I am responsible for all FEES/MONIES related to the season. I fully acknowledge that if I DO NOT participate in / turn in the required game work and fundraisers by due dates, my player may be held out of games.
