WAIVER INFORMATION

As parent or guardian of the child indicated on this form, I grant my permission for this child’s participation in the KYAA Baseball program for the 2025 season. I understand that inherent in these sporting activities is some risk of physical injury, and I release the KYAA and its officers, directors and coaches from liability for injury incurred in all league activities. I also grant my permission to the KYAA and its representatives to secure any and all emergency medical treatment required by my child in my absence.


CODE OF CONDUCT AND SPORTSMANSHIP FOR PARENTS, PARTICIPANTS AND FANS

Any player, parent, guest, fan or any person in attendance at any Kutztown Youth Athletic Association practice, game or event:

- WILL NOT ride, abuse, heckle or yell in a nonproductive manner to any other player, parent, coach, staff member, guest, fan, board member, opposing coach, opposing player, opposing team?s fans or game official
- WILL NOT attempt to influence any decision made by any game official
- WILL NOT bait or taunt any opposing player, coach or game official
- WILL NOT attempt to incite a nonproductive reaction by fans through the use of words or actions
- WILL NOT come onto the playing/practice field without the direct authorization of the head coach or another KYAA official or officer
- WILL NOT continue to do anything after being informed that any action or words are inappropriate by the head coach or any KYAA board member
- WILL NOT be under the influence of drugs or alcohol at any time during any practice or game

Anyone who is found to be in violation of this policy will be disciplined by the board and furthermore, any and all children attached to said adult will not be allowed to participate further until said discipline is lifted. There will be NO exceptions to this code. It should be clearly understood that unsportsmanlike conduct has no place in our youth program and will not be tolerated in any way, at any time, for any reason.

By checking the box below I do hereby understand and will conform to the code of conduct and sportsmanship rules as stated above. I also understand that it is my responsibility to inform all of my participating children and any guest I may bring of this code.


PHOTO RELEASE CONSENT

I hereby give KYAA consent to record and photograph my athlete during KYAA games and events for promotional purposes.
