This form can not be used for individual payment of registration. You must list and pay for a minimum of 3 players. If you only register 3 players on this form and later add a 4th or 5th player you MUST contact Meri Dickens to add players and arrange payment for their entry fee. Only players listed on your team's registration will be allowed to play in the tournament.
