WAIVER INFORMATION

I, the undersigned, am the parent or legal guardian of the above-named participant in Milbank Youth Boys' Basketball. In consideration of my child's participation in these activities, I acknowledge and agree to the following:
1. Assumption of Risk: I understand that participation in basketball activities involves inherent risks of injury, including but not limited to, sprains, fractures, and concussions. I voluntarily assume all risks associated with such participation.
2. Health and Medical Insurance: I certify that my child is in good health and has medical insurance that covers any injuries sustained while participating in Milbank Youth Boys' Basketball activities.
3. Consent to Medical Treatment: In the event of an injury or medical emergency, I authorize Milbank Youth Boys' Basketball and its representatives to seek and consent to medical treatment on behalf of my child.
4. Release and Waiver: I release Milbank Youth Boys' Basketball, its coaches, volunteers, and officials from any liability for personal injury, property damage, or wrongful death that may occur during participation in youth basketball activities. This release applies to all activities organized or sanctioned by Milbank Youth Boys' Basketball.
5. Photography and Likeness: I grant Milbank Youth Boys' Basketball permission to use photographs, videos, and likenesses of my child for promotional or educational purposes.
6. Behavior and Code of Conduct: I understand that my child is expected to adhere to the organization's code of conduct, and any violation may result in disciplinary action or expulsion from the program.
7. Parental Responsibility: I will ensure that my child arrives and departs from Milbank Youth Boys' Basketball activities safely and promptly. I will also adhere to any additional rules and guidelines set forth by Milbank Youth Boys' Basketball.
