2026 Team Registration Form


Please fill out this form for each of the teams you have playing in the league for 2026. Once you click "Submit Form" there will be a button to add additional teams. Make sure to click "CHECKOUT" once all the forms have been submitted.

Please check with your town rep prior to submitting the form to make sure they haven't already submitted it on your behalf.

All lines marked with an * are required to submit the form.

League Dues are $75 per team and are due by March 1st.

League Tournament Fees are $200.

The information you list for the head coach is what will be used for the contact info for scheduling. Once scheduling is complete if you want to update the head coach please send an email of what you want updated. Do not use apostrophes for your team names, I can change them later if needed after scheduling is complete.
