In case of illness or injury resulting from these activities, I hereby waive all claims against the Organization, Sponsors, Coaches, Directors, Officers, and the Town of Norwood. I also release from responsibility any person (s) transporting my child to or from any of these activities. I understand my responsibility towards my child's medical expenses if any occur. I further give my consent for medical treatment by the closet hospital, doctor or medical facility.

For the 2nd player complete the Form above including 'Select Fee' - choose $250 and click 'Submit'. Once the next screen appears - you are finished.
