2026 SASHL Wed Summer Player Registration Form

2026 SASHL Wed Summer Player Registration Information:

Must be 25 years or older on or before 12/31/2026 to play (unless board approved).
Must be current and registered with USAHOCKEY.COM (https://membership.usahockey.com), and provide USAH Reg # on registration form for Current season 2025-26 is valid, or Newer 2026-27 reg opens up after Apr 23rd, 2026. (also see this registration form page for details).
Note this is a separate fee paid to USA Hockey and is REQUIRED to play.

PAYMENT INFORMATION

$300 League Fee: Full Payments due before play begins or sooner. Payment Deadline is Monday, June 1st, 2026. First Game is Wed, June 3rd, 2026.
If not paid by deadline, you will not be guaranteed a spot!

WAIVER OF LIABILITY
I certify that I am cognizant of all the inherent dangers, risks, and hazards associated with ice hockey. In consideration of being permitted to participate in the 2026 Wednesday Summer Sylvania Area Senior Hockey League (SASHL), I hereby voluntarily assume all risks of accident or injury to my person or property, whether foreseen or unforeseen.

I am currently registered with USA Hockey as player for 2025-26 or 2026-27 season and will provide such current registration w/USA Hockey to league or Tamo prior to 06/03/2026. If I do not provide current USA Hockey reg, I will not be able to play nor receive refund for any fees paid to SASHL.

I hereby release SASHL and TAM-O-SHANTER ice arena and their employees, agents, and its representatives from any claim, liability, demand, or suit of any kind sustained, whether or not caused by the negligence of SASHL and its representatives.

I further agree to indemnify and hold SASHL and TAM-O-SHANTER ice arena and their employees, agents, and its representatives harmless from any claim, liability, demand, or suit arising out of any alleged malfeasance, misfeasance, or nonfeasance arising in connection with the 2025 SASHL Wednesday Summer Session. This release shall be binding upon my heirs, administrators, executors and assigns.

I represent that I am of lawful age and legally competent to consent this release; that I understand that the terms herein are contractual; and that I have agreed to the terms of this document as my own free act.
