Baseball Sign-Up Form

In consideration of being allowed to participate in any way in Alba Golden Youth Sports Foundation, Inc. (AGYSF) athletic/sports program, and related events and activities, the undersigned:

1. Agrees that the parent(s), and/or legal guardian(s) will do and will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant or parent(s), and/or legal guardian(s) believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition and refuse to participate. The parent(s), and/or legal guardian(s) are hereby encouraged to purchase any and all equipment for the protection of the minor participant. Any equipment provided by AGYSF is on a voluntarily basis, it is the parent(s), and/or legal guardian(s) responsibility to ensure proper fit, function and to maintain the equipment so that it provides adequate protection.

2. ACKNOWLEDGES AND FULLY UNDERSTANDS that each participant will be engaging in ACTIVITIES THAT INVOLVE RISK OF SERIOUS INJURY, INCLUDING PERMANENT DISABILITY AND DEATH, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the action, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonable foreseeable at this time.

3. ASSUMES ALL THE FOREGOING RISK AND ACCEPT PERSONAL RESPONSIBILITY FOR THE DAMAGES following such injury, permanent disability or death.

4. RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE Alba Golden Youth Sports Foundation, Inc., its respective administrators, directors, agents, volunteers, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, Alba Golden ISD, its affiliated clubs and their agents, all of which are hereinafter referred to as “releases”, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES ON ACCOUNT OF INJURY, INCLUDING DEATH OR DAMAGE TO PROPERTY, CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PARTY BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

5. I understand that it is my responsibility to transport the participant to and from practices and games. In the event I allow the participant to be transported by another party, I hereby release and hold harmless said party from any loss or injury that may be sustained by the participant while riding with said party to and from practices and/or games.




Parent and Player Code of Conduct:

We, the Alba-Golden Youth Sports Foundation (AGYSF), have implemented the following Parent and Player Code of Conduct for the important message it holds about the proper role of parents in supporting their child in sports. Parents must read, understand, and agree to this form prior to their children participating in our league.
Any parent or player guilty of improper conduct at any game or practice will be asked to leave the sports facility and be suspended from the following game. Repeat violations may cause a multiple game suspension, or the season forfeiture of the privilege of attending all games.

The essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship. Here at AGYSF, all participants including parents, coaches, players, volunteers, and members of the foundation, will agree and adhere to these core principles:

* Respect
* Trustworthiness
* Fairness
* Humility
* Responsibility
* Effort
* Attitude
* Character
* Class

The highest potential of sports is achieved when competition reflects these principles. Children who wish to play, parents who want their children to play, and Coaches who wish to coach on teams sanctioned by AGYSF are required to understand and adhere to this Code of Conduct fully. I therefore agree:

1. I will not force my child to participate in sports.
2. I will and my child will follow the direction of the coaching staff and show respect for all opponents, coaches, players, umpires, league officials and spectators at all times.
3. I will remember that children participate to have fun and that the game is for youth, not adults.
4. I will refrain from coaching my child or other players during games and practices unless I am one of the official coaches of the team.
5. I will respect the officials and coaches and their authority during games. I will never question, discuss, or confront coaches on the game field, and will take time to speak with coaches at an agreed upon time and place.
6. I will learn the rules of the game and the policies of the league.
7. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice, or other sporting event.
8. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures.
9. I will not encourage any behaviors or practices that would endanger the health and wellbeing of the athletes.
10. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.
11. I will demand that my child treat other players, coaches, officials, and spectators with respect regardless of race, creed, color, sex, or ability.
12. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.
13. I will promote the emotional and physical well-being of the athletes ahead of any personal desire I may have for my child to win.

The Alba-Golden Youth Sports Foundation Board of Directors reserves the right to impose penalties, including, without limitation, game suspensions, dismissal from the team, suspension, or revocation of the opportunity to participate in additional youth sports leagues and(or) seasons for serious or repeated violation of this Code of Conduct or other conduct deemed to be harmful to AGYSF or any affiliated people or organizations. We agree to abide by the rules and regulations stated herein, and we are bound to all decisions and impositions of these rules. It is with the spirit of cooperation, for the betterment of youth sports and our community, that we willingly abide by this contract.



Concession Stand Duty:
In order for there to be a successful youth sports program in our community, there must be a commitment of selfless volunteers. Each team will be given designated day(s) and times where they are expected to provide volunteer resources. If for some reason those times are not handled, the team responsible will be at risk of forfeiting the next scheduled game. If you or anyone you know would like to volunteer more than what is asked of your team, please let your coach or a member of the AGYS Foundation know. Each and every hour is appreciated!



Players will play in the division of their age group. If requesting to move up a division, please notate that in the additional comments section below.
Accommodations are not guaranteed but will be considered if there is no negative impact on the age groups effected.
